Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

Dr. Lance Henry is a board-certified dermatologist providing comprehensive medical and cosmetic care to patients at the Advanced Dermatology and Skin Cancer Center since 2009. Dr. Henry also teaches Dermatology residents and serves as an attending physician at the VA Medical Center/University of Arkansas for Medical Sciences.
Dr. Henry received his medical degree from the University of Arkansas for Medical Science, where he was a member of the Alpha Omega Alpha Medical Honor Society and the Associated Student Government Treasurer. He received the Barton Foundation Scholarship for having the highest grade point average in his medical school class.
Upon completion of his degree, Dr. Henry completed his internship and residency training in Dermatology at the University of Arkansas as well, serving as Chief Resident during his final year. He also completed a fellowship in Mohs Micrographic Surgery/Procedural Dermatology at the University of Rochester Medical Center.

Andrea Thompson graduated cum laude from Western Connecticut State University in 1998 with a Bachelor of Arts degree in Biology. She then completed a research fellowship in Pediatrics at Danbury Hospital, and also served as a Patient Care Technician in the Danbury Hospital Department of Emergency Medicine Department.
Ms. Thompson is a graduate of the Physician Assistant Program at Quinnipiac University in Hamden, Connecticut, and is certified by the National Commission on Certification of Physician Assistants. She is a member of the American Academy of Physician Assistants, the Connecticut Academy of Physician Assistance and the Society of Dermatology Physicians Assistants. Ms. Thompson was the co-founder and treasurer of the Connecticut Dermatology Physician Assistant Society.
She joins our team with nine years of dermatology experience.
