Patient Story: Large Basal Cell Carcinoma on the Nose Treated with Staged Mohs Surgery and Surgical Reconstruction
A 78-year-old female patient presented with a large basal cell carcinoma (BCC) on her nose, a sensitive and highly visible location that required advanced care. Due to the size of the tumor and its proximity to delicate nasal structures, Dr. Lance Henry, double board-certified dermatologist and micrographic surgeon, and the most experienced Fellowship-trained Mohs Surgeon in the region, performed Mohs micrographic surgery to ensure complete cancer removal while preserving healthy tissue.
This extensive case required three stages of Mohs surgery to fully eliminate the tumor. Each stage involved the careful removal and immediate microscopic analysis of cancerous tissue, a process repeated until all margins were clear. Because of the significant amount of tissue removed and the functional and cosmetic importance of the nasal region, a follow-up surgical procedure was necessary to close the defect and reconstruct the area.
Dr. Henry designed a customized reconstruction to restore the natural shape and contour of the nose. His approach prioritized structural support, airflow preservation, and skin blending. At eight weeks post-reconstruction, the “after” photo shows smooth healing, maintained nasal symmetry, and a well-integrated repair that respects the patient’s facial aesthetics.
This case highlights the full scope of Dr. Henry’s expertise—not only in advanced Mohs surgery, but also in surgical planning and complex facial reconstruction. The patient recovered well and expressed great satisfaction with both the medical and cosmetic results.
