Club Revive benefits are only for the member and only at the Harrison location. The member must schedule their treatments each month, or they will be waived. Club Revive program is automatically renewed at the end of each enrollment term. Benefits can be canceled after the initial term of membership is complete by giving 30-day written notice to Revive Medical Spa, LLC. Early cancelations are subject to an early termination fee equal to the remaining months on the contract times $125.
