Financial Policy & Notice of Privacy Practices
To help us keep appointments running on time, please:
Arrive 15 minutes before your scheduled appointment, unless asked to do differently when scheduling the appointment.
Bring your insurance information and photo ID.
Complete your paperwork in advance whenever possible.
Upload a photo of your insurance card through the online registration link if prompted.
Need directions?
We respect your time and the time of every patient we serve.
Patients who arrive more than 10 minutes late may need to reschedule their appointment.
Depending on provider availability, a late patient may be offered the opportunity to:
Be seen as a same-day walk-in on a first-come, first-served basis, or
Reschedule for another available appointment.
Completing your paperwork before arrival helps speed up the check-in process and gives us the best opportunity to keep your scheduled appointment.
If you need to cancel or reschedule your appointment, please notify us at least 24 business hours in advance.
Examples:
Wednesday appointment → cancel by end of day Monday
Monday appointment → cancel by end of day Friday
We send appointment reminders by text and email when notifications are enabled.
Fees
A $100 cancellation/no-show fee applies to:
Cosmetic Dermatology appointments
Mohs Surgery appointments
Dermatologic Surgery appointments
For Pearl, Ultra RF, and Juliet treatments:
Less than one full business day’s notice or a no-show will result in 50% of the prepaid treatment fee being retained.
Please note:
Insurance does not cover cancellation or no-show fees.
Three no-shows within a one-year period may result in dismissal from the practice.
We understand that unexpected situations happen. Please contact us as soon as possible if you are unable to keep your appointment.

A $100 prepaid consultation fee is required for cosmetic consultations and appointments.
This fee:
Applies toward injectable or laser treatments purchased following your consultation.
Is non-refundable.
Is forfeited if you cancel with less than 24 business hours’ notice or fail to attend your appointment.
Cosmetic procedures must be prepaid before they are scheduled.

Before your procedure, our billing team will verify your insurance benefits and notify you of your estimated financial responsibility.
A surgery deposit is required:
Five (5) business days before surgery, or
Immediately if the required deposit is $100 or less.
Outstanding account balances must be paid before additional appointments can be scheduled.

A $25 fee applies to returned checks due to insufficient funds or closed accounts.
Credit Card Surcharge
Effective January 5, 2026, a 3% surcharge applies to major credit card transactions.
Debit cards, HSA cards, and FSA cards are not subject to this surcharge.
Patients without insurance are required to prepay for services before an appointment can be scheduled.
If you have a biopsy, pathology services may generate a separate bill.
Depending on your diagnosis, billing may come from:
Advanced Dermatology & Skin Cancer Center, or
An outside pathology laboratory.
Payment is due at the time of purchase.
All product sales are final.
Returns and exchanges cannot be accepted.

$25 labor fee
$0.50 per page for the first 25 pages
$0.25 per page after the first 25 pages
Postage (if mailed)

Payment must be received before records are processed.
