Patient Story: Large Basal Cell Carcinoma on the Scalp Treated with Mohs Surgery
An 86-year-old male patient was diagnosed with a large basal cell carcinoma (BCC) on the scalp—a sun-exposed area where tumors can grow extensively before being detected. Due to the size and location of the lesion, Dr. Lance Henry, double board-certified dermatologist and micrographic surgeon, and the most experienced Fellowship-trained Mohs Surgeon in the region, recommended Mohs micrographic surgery for complete and accurate removal followed by careful reconstruction.
Mohs surgery is the gold standard for high-risk or large-area skin cancers like this one. It allows for layer-by-layer removal of the tumor, with each tissue section examined under a microscope until all cancerous cells are cleared. This technique ensures complete excision while minimizing the removal of healthy surrounding tissue—particularly important on the scalp, where skin mobility is limited and protection of the underlying bone is critical.
Once the cancer was removed, Dr. Henry reconstructed the area to support healthy healing, close the surgical site securely, and prevent tension that could interfere with regrowth. At six weeks post-surgery, the “after” photo shows excellent healing: healthy tissue, reduced inflammation, and solid closure with early signs of hair regrowth near the site.
This case reflects the challenges of treating scalp BCCs in older adults, including thinner skin, limited elasticity, and increased healing time. With Dr. Henry’s advanced surgical skill and experience, the patient achieved a safe recovery, full cancer clearance, and a durable, cosmetically acceptable result.
