Scheduling is easy!

A valid credit card is required to reserve all appointments.
Please arrive 15 minutes before your scheduled appointment.
This gives you time to complete any necessary paperwork and allows your treatment to begin on time.
Arriving 15 minutes early is considered arriving on time.
If you arrive late, we will do our best to accommodate you; however, your appointment may need to be shortened or rescheduled to avoid impacting other clients.
If you’re running behind, please call us as soon as possible so we can discuss your options.
Your safety is always our priority.
If you are pregnant or breastfeeding, please let us know before scheduling or receiving treatment so we can recommend services that are appropriate for you. We also encourage you to consult your physician before beginning any spa treatment.
Cosmetic dermatology consultations require a prepaid, non-refundable deposit. This deposit may be applied toward the treatment recommended during your consultation.
We understand that schedules change. If you need to cancel or reschedule, please let us know as early as possible.
One full business day notice required.
Appointments must be canceled or rescheduled at least one full business day before your appointment.

Appointments canceled with less than one full business day’s notice will be charged 50% of the scheduled service fee.
Missed consultations or follow-up appointments are subject to a $35 late cancellation fee.
A $100 prepaid consultation fee is required for cosmetic consultations.
Because these treatments require extended appointment times and preparation, 50% of the prepaid treatment fee will be retained for appointments canceled with less than one full business day’s notice or for no-shows.

Due to updated regulations from the Arkansas State Board of Pharmacy and the Arkansas Department of Health, prescription-strength topical numbing cream can no longer be delivered directly to medical clinics.
If your treatment requires prescription numbing cream, you will need to fill your prescription through a compounding pharmacy before your appointment.
Our team will guide you through every step of the process and help make it as easy as possible.
We appreciate your understanding as we comply with these state regulations.
All services are non-refundable.
Defective products may be exchanged within 5 days of purchase.
Creating a peaceful environment is important to us and to every guest visiting Revive.
Please keep conversations at a respectful volume to maintain a relaxing atmosphere.
Please silence your phone during your visit.
If you need to take a call, we kindly ask that you step into the lobby.
Children may only be present if they are receiving a scheduled service.
Clients under the age of 16 must be accompanied by an adult.
A parent or guardian must complete a consent form for all clients under 18.
Gratuities are never expected but are always appreciated.
Tips may be given directly to your provider or added at checkout. Gratuity envelopes are available for your convenience. Most providers also have a Venmo link.
If your visit includes locker access, please keep your assigned locker key with you throughout your appointment.
Lost or unreturned keys will incur a $25 replacement fee, which will be charged to the card on file.

Payment is due at the time of service.
Returned checks are subject to a $25 returned check fee.
Effective January 5, 2026, a 3% surcharge will be applied to all major credit card transactions. Debit cards and HSA/FSA cards will be exempt from this surcharge.
Our providers and team members are committed to creating a safe, respectful, and welcoming environment for every guest.
Any inappropriate, threatening, or disruptive behavior toward staff or other clients will result in the immediate end of the appointment. The full-service fee will be charged, and the individual may be prohibited from returning to any Revive Medical Spa location.
